Job summary

ComplexCare Solutions is seeking a Nurse Practitioner to work per diem, providing comprehensive in-home and telehealth assessments across the region. You will spend up to an hour with each member to understand personal needs and coordinate care to keep them in their homes and out of the hospital.

The role supports care plans, adheres to CMS/HEDIS guidelines, documents findings, and communicates with PCPs and health plans.

Qualifications

• Active un-encumbered license to practice nursing.
• ANCC or AANP board certification as NP/CS in Family, Adult, Gerontology or Emergency Medicine is required.
• Maintains current CPR certification.

Responsibilities

• Know ledge of CMS Regulations and NCQA HEDIS Guidelines.
• Complete comprehensive review of the assigned member population and timely documentation.
• Document active medical conditions in medical records per CMS/DHHS and client requirements.
• Communicate findings to PCPs/health plans and coordinate follow-up.
