Qualifications

• Active un-encumbered nursing license.
• ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine.
• Current CPR certification.
• Vaccination and preventive testing as per policy.
• Home health experience a plus.
• Excellent communication with elderly and chronically ill patients.
• Ability to practice autonomously in a remote environment.
• Proficient with computer platforms and EHRs.

Responsibilities

• Conduct comprehensive home and telehealth assessments for members.
• Document and communicate assessment findings, treatment plans and follow up needs.
• Identify gaps in care and coordinate with PCPs and health plans.
• Maintain HIPAA-compliant documentation and data security.
