Job summary

ComplexCare Solutions is seeking a Nurse Practitioner per diem to perform comprehensive in-home and telehealth assessments across San Joaquin County, CA. You will document findings, coordinate with PCPs and health plans, and help close gaps in care while adhering to CMS and HIPAA requirements.

Candidates must hold ANCC or AANP NP board certification and an active NP license. Remote clinical work with potential to travel for in-home visits may be involved, with a focus on improving outcomes for

Qualifications

Active, un-encumbered NP license required and board certification in Family, Adult, Gerontology or Emergency Medicine.
Experience with home health or remote assessments is beneficial.
Ability to conduct comprehensive member assessments and document findings accurately.

Responsibilities

Conduct comprehensive in-home or telehealth member assessments for care gaps and needs.
Document assessment findings, recommendations, and follow-up plans in the medical record and CMS-compliant format.
Support physician/patient relationships with timely communication and documentation.
Manage chronic conditions and develop evidence-based treatment plans remotely.
Ensure HIPAA compliance and maintain confidentiality of all data.
