Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for per diem in Kern County, CA. The role involves in-home assessments and remote patient engagement to evaluate health status, gaps in care, and care management opportunities to improve outcomes.

The provider will work with a focus on elderly and chronically ill populations. Candidates must hold ANCC or AANP certification, maintain an active nursing license, and be proficient with EHRs.

Qualifications

• Active NP license in good standing.
• ANCC or AANP board certification in relevant family/adult/gerontology/emergency specialty.
• Experience with home health or remote patient assessments is a plus.
• Ability to document thoroughly and comply with CMS/HIPAA requirements.
• Proficient with EHRs and secure messaging; capable of autonomous, remote assessments.

Responsibilities

• Conduct comprehensive in-home or telehealth NP assessments (approx. 60 minutes).
• Develop evidence-based treatment plans and care recommendations.
• Document findings and coordinate follow-up with PCPs and health plans.
• Identify and address gaps in care and potential health risks.
• Maintain HIPAA-compliant records and adhere to company policies.
