Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for a Per Diem role covering Merced County, CA. The NP will conduct comprehensive in-home and telehealth assessments, document findings, and develop care plans to support staying out of hospital

The role requires an active NP license with board certification and the ability to work autonomously in a remote clinical environment while ensuring CMS/HEDIS compliance and high-quality patient care.

Qualifications

• Active un-encumbered NP license.
• Board certification in Family, Adult, Gerontology or Emergency Medicine.
• Knowledge of CMS Regulations and NCQA HEDIS guidelines.
• Proficient with EHRs and secure messaging.

Responsibilities

• Conduct comprehensive member assessments in home and telehealth settings.
• Document and communicate assessment findings, treatment plans and follow-up needs.
• Escalate urgent cases per policy and professional judgment.
• Address gaps in care and provide patient education on chronic conditions.
• Maintain HIPAA-compliant documentation and confidentiality.
