Job summary

ComplexCare Solutions (CCS) is seeking a Nurse Practitioner for per diem in Oregon to conduct in-home and telehealth assessments. You will review medical histories, document encounters, and coordinate with primary care providers and health plans to optimize patient care.

The role emphasizes CMS/HEDIS knowledge, independence in remote practice, and delivering patient-centered care with a focus on chronic disease management. Competitive compensation and benefits are offered.

Qualifications

• Active un-encumbered NP license required.
• ANCC or AANP board certification in appropriate specialty.
• Current CPR certification.
• Vaccination and preventive testing compliance prior to hire (Hepatitis B, Flu, MMR, Tdap, Varicella, TB).
• Home health experience a plus.

Responsibilities

• Perform comprehensive home assessments and document findings.
• Maintain CMS/DHHS-compliant documentation and coding.
• Communicate assessment findings and care recommendations to PCPs and health plans.
• Identify and close gaps in care and provide patient education.
• Provide autonomous care in remote clinical settings and manage chronic conditions.
