Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for Per Diem coverage across home and virtual assessments. You will spend time with members to understand personal needs and keep them out of the hospital. The role supports clinicians and plans nationwide and requires strong documentation and communication.

Active NP license with ANCC or AANP certification and CPR are mandatory. Remote work with flexibility to meet monthly slot targets, and HIPAA-compliant practice are essential.

Qualifications

• Active, un-encumbered nursing license and NP board certification required.
• ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine.
• CPR certification and HIPAA compliance knowledge.
• Experience with CMS/NCQA guidelines and HEDIS familiarity preferred.
• Ability to document and communicate assessment findings clearly.

Responsibilities

• Conduct comprehensive home and telehealth assessments for members.
• Document findings, create evidence-based treatment plans and follow-up plans.
• Coordinate with PCPs and health plans and close care gaps.
• Maintain HIPAA compliance and accurate medical records.
• Operate autonomously in a remote setting and manage chronic conditions.
