Job summary
ComplexCare Solutions is seeking a Nurse Practitioner for per diem roles to perform comprehensive home and telehealth assessments across Lane County, OR and beyond. The position emphasizes patient engagement, evaluating health status, identifying gaps in care, and developing action plans to improve outcomes.
Offered sign-on bonus of 3,500 and a pay range reflecting in-home visit rates; telehealth assessments may apply.
Qualifications
- Active NP license
- ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine
- CPR certification is up to date
- Knowledge of CMS Regulations and NCQA guidelines
- Familiarity with ICD-10 and CPT-4 coding practices
- Ability to conduct comprehensive patient assessments and document findings
- Experience with in-home health or telehealth settings is a plus
Responsibilities
- Conduct patient assessments in home or telehealth settings
- Document findings and recommendations in the medical record
- Coordinate with PCPs and health plans for follow-up
- Address gaps in care and provide evidence-based treatment plans
- Maintain HIPAA-compliant communication and documentation
- Manage multiple patient visits and scheduling requirements
- Perform point-of-care testing and report results
- Uphold CMS/DHHS and client policies in all actions
