ComplexCare Solutions is seeking a Nurse Practitioner for per diem work to deliver in-home and telehealth assessments for health plans nationwide. The role emphasizes capturing a comprehensive view of each member’s needs and coordinating with PCPs and care teams.
The ideal candidate holds an active NP license, ANCC/AANP board certification in Family/Adult/Gerontology/Emergency, and CPR certification, with strong documentation skills and HIPAA compliance for home-based care.
Qualificações
- Active unencumbered NP license required.
- ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine.
- Maintains current CPR certification.
- Knowledge of CMS Regulations and NCQA HEDIS Guidelines is advantageous.
- Excellent documentation and communication skills.
Responsabilidades
- Provide comprehensive in-home assessments and care planning.
- Coordinate with PCPs and care teams for follow-up and referrals.
- Document encounters accurately in EHR and ensure HIPAA compliance.
- Manage chronic conditions through evidence-based treatment plans.
- Maintain up-to-date knowledge of CMS, Medicare, and health plan benefit structures.
