ComplexCare Solutions is seeking a Nurse Practitioner for per diem assignments primarily serving Kern County Northwest, CA. You will conduct comprehensive home assessments, address chronic conditions, and collaborate with PCPs and health plans to improve patient outcomes.

Active NP license with ANCC/AANP board certification is required, along with CPR and vaccination compliance. Role may include telehealth and in-home visits with a focus on autonomous practice in a remote setting.

Qualificações

• Active unencumbered NP license to practice in the state.
• ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine.
• CPR certification and compliance with vaccination/testing requirements.

Responsabilidades

• Conduct comprehensive member assessments and timely documentation.
• Coordinate with physicians and health plans to ensure appropriate follow-up.
• Identify and close gaps in care (disease- or preventive-focused).
• Document assessment findings, recommendations, and follow-up timelines.
• Perform Point of Care testing and communicate results.
• Maintain HIPAA compliance and protect patient data.
