ComplexCare Solutions in the United States (Shasta County, CA) seeks a Nurse Practitioner per diem to conduct comprehensive home and telehealth assessments, spend about an hour with each member, and develop evidence-based treatment plans that keep patients in their homes and out of the hospital.

Requires ANCC or AANP certification, active RN license, ability to work remotely, maintain HIPAA compliance, and provide thorough documentation.

Qualifications

• ANCC or AANP certification as Nurse Practitioner and active RN license are required.
• Experience in home health or chronic care management is a plus.
• Ability to work autonomously while documenting care accurately and timely.

Responsabilités

• Perform and document comprehensive member assessments and develop evidence-based treatment plans.
• Provide care in home and telehealth settings to keep members out of hospital.
• Communicate results and coordinate with PCPs and health plans as required.
• Maintain HIPAA compliance and accurate medical record documentation.
