Job summary

SCAN is seeking a Manager of Care Coordination to lead teams delivering education and navigation assistance for members, ensuring compliance with regulatory standards.

You will supervise workflow, recruit and train staff, and maintain high-quality care coordination services across multiple states. The role emphasizes collaboration with stakeholders, data-driven improvement, and adherence to NCQA and CMS guidelines.

Qualifications

• Bachelor's Degree or equivalent experience.
• 5+ years of experience in healthcare, medical management, change management, and/or leadership.
• Demonstrated management experience.
• Must possess working knowledge of NCQA standards, SNP requirements, and CMS/DHS regulations.
• Knowledge of project and program management principles.
• Ability to develop and implement new systems and gain buy-in.
• Able to lead and influence across reporting lines.

Responsibilities

• Manages teams and day-to-day operations to align with department goals and objectives.
• Oversees workflow, prioritizes projects, and ensures timely completion of activities.
• Recruits, trains, and maintains staff in a safe, secure work environment.
• Communicates expectations, coaches, and enforces policies and procedures.
• Ensures policy procedures and regulatory requirements reflect Care Coordination practices.
• Leads meetings and trains staff to deliver care coordination per SCAN policy.
• Represents department on committees and supports escalation of issues.
• Develops department metrics and analyzes monthly reports to drive outcomes.
• Assists Director in annual program evaluation and related activities.
