Job summary

Cadence Health is seeking a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will conduct proactive outreach, coaching, and care coordination to close gaps, educate patients, and address barriers in a remote, patient-centered model.

The role requires active LPN/LVN licensure, 5+ years of clinical experience, and strong communication skills.

Qualifications

• Active LPN/LVN license in multiple states is required.
• Minimum 5 years of clinical experience as an LPN/LVN.
• Remote work readiness with stable high-speed WiFi is required.
• Active CA state LVN license is required.
• Fluency in Spanish is preferred.
• Experience providing patient education and support to individuals with chronic conditions is preferred.
• Familiarity with AI-assisted clinical tools and discussing AI use with providers and patients is a plus.

Responsibilities

• Conduct outbound telephonic outreach and provide one-on-one coaching to patients managing chronic conditions.
• Educate patients on personalized care plans and support behavior change, nutrition, activity, and self-management.
• Perform comprehensive health assessments and evaluate readiness to change.
• Educate on disease management, medication adherence, symptoms, and preventive strategies, following Cadence policies.
• Monitor progress, adherence, and outcomes with remote tools.
• Document interactions and follow-up plans; connect patients with resources as needed.
• Collaborate with clinicians and support teams using Cadence Platform, Google Workspace, and Slack.
