Job summary

Cadence is a clinical AI company delivering virtual chronic care support to Medicare patients. The role targets a Licensed Practical Nurse for remote, full-time work in the United States, with standard hours and access to AI-enabled clinical tools.

You will conduct patient outreach, educate on self-management, and coordinate care plans across multiple systems, while adhering to Cadence policies and clinical guidance.

Qualifications

• Active compact multi-state LPN/LVN license required.
• Minimum 5 years of clinical experience as an LPN/LVN.
• Remote work readiness with reliable high-speed Internet.
• CA state LVN license highly preferred.
• Spanish fluency strongly preferred.
• Experience educating and supporting patients with chronic conditions.
• Experience speaking credibly about AI-supported care with providers and patients.

Responsibilities

• Conduct outbound telephonic outreach and one-on-one coaching for chronic conditions.
• Educate and coach patients on behavior modification, nutrition, activity, and self-management.
• Assess patients' health status, lifestyle, and readiness to change.
• Educate on disease management, medication adherence, and prevention; follow Cadence protocols.
• Monitor progress and outcomes via remote tools; document interactions.
• Collaborate with clinicians and support teams in real time using Cadence Platform, Google Workspace, and Slack.
