Job summary

Cadence, a clinical AI company, seeks a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This is a full-time, remote role based in the United States with standard hours.

You will conduct outbound outreach, coach patients on behavior change and self-management, document care activities, and collaborate with clinicians using Cadence Platform, Google Workspace and Slack.

Qualifications

• Active compact multi-state LPN/LVN license in the state of residence.
• Minimum of 5 years of clinical experience as an LPN/LVN.
• Remote work readiness with stable high-speed WiFi.
• CA LVN license preferred.
• Spanish language proficiency strongly preferred.
• Experience educating and supporting patients with chronic conditions.
• Familiarity with AI-assisted clinical tools and ability to discuss AI-supported care vs. human judgment.

Responsibilities

• Outreach to patients by phone and provide coaching for chronic conditions.
• Guide patients in executing personalized care plans and self-management strategies.
• Assess health status, lifestyle, and readiness to change.
• Educate on disease management, medication adherence, and prevention.
• Monitor progress and health outcomes through remote tools.
• Document interactions and connect patients with resources.
• Collaborate with clinicians using Cadence Platform, Google Workspace, and Slack.
