Job summary

Cadence seeks a Clinical Navigator (LPN/LVN) for remote Medicare patient support. You will lead coaching, care coordination, and education to close care gaps while documenting interactions in the Cadence platform.

Collaboration with clinicians and a remote team will be essential to deliver proactive, patient-centered care. Ideal candidates have 5+ years of LPN experience, active multi-state licensure, and strong communication skills.

Qualifications

• Active multi-state LPN/LVN license required.
• At least 5 years of clinical experience as an LPN.
• Remote work readiness with stable high-speed WiFi.
• Active CA LVN license required.
• Spanish fluency preferred.
• Experience educating and supporting patients with chronic conditions.
• Familiarity with AI-assisted clinical tools and collaborating with providers.

Responsibilities

• Perform outbound telephonic outreach and one-on-one coaching for chronic disease patients.
• Assist patients in executing personalized care plans with behavior modification education.
• Conduct comprehensive health assessments and readiness-to-change evaluations.
• Educate on disease management, adherence, and prevention per policies.
• Monitor progress and document interactions in the Cadence platform.
• Coordinate with clinicians and support teams in real time.
