Job summary

Cadence is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This is a full-time remote role based in the United States with standard Pacific/Mountain hours.

You will conduct outbound outreach, educate patients on chronic disease management, and collaborate with clinicians using Cadence Platform, Google Workspace, and Slack. LPN/LVN license and 5+ years of clinical experience required.

Qualifications

Active LPN/LVN license in multi-state compact is required.
Minimum 5 years of clinical LPN/LVN experience required.
Remote-work readiness with reliable WiFi is required.
Spanish fluency strongly preferred.
Experience educating patients with chronic conditions preferred.

Responsibilities

Conduct outbound telephonic outreach and one-on-one coaching for chronic conditions.
Educate and coach patients on behavior modification, nutrition, activity, and self-management using evidence-based techniques.
Perform comprehensive health and lifestyle assessments.
Educate patients on disease management, medication adherence, symptoms, and prevention.
Monitor progress and health outcomes via remote tools and check-ins.
Document interactions, outcomes, and follow-ups in the Cadence platform.
Collaborate with clinicians and support teams using Cadence Platform, Google Workspace, and Slack.
