Vynca Inc. is seeking an experienced Lead Care Manager to coordinate client care with healthcare providers, social service agencies, and family support teams. The role emphasizes person-centered planning, education, and timely access to services to maximize member benefit and independence.

The Lead Care Manager will manage cases, coordinate benefits, and ensure quality outreach across diverse populations, with a focus on compassionate, compliant care delivery and cross-functional teamwork.

Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
• Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for potential evenings and weekends.
• Working knowledge of government and community resources related to social determinants of health.
• Clean driving record, valid driver's license, and reliable transportation.
• Bilingual (English/Spanish).

Responsibilities

• Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referrals to community-based services.
• Oversee development of client care plans and goal setting.
• Offer services where the member resides, including office-based, telehealth, or field-based.
• Connect clients to social services and supports as needed.
• Advocate on behalf of the client with health care professionals.
• Utilize Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles.
• Conduct outreach to facilitate linkage to the ECM program and log activity in the CRM.
• Evaluate client progress and update SMART goals.
• Provide mental health promotion.
• Arrange transportation (e.g., ACCESS).
• Complete documentation and outcome measures within care plans timelines.
• Maintain up-to-date patient health records in EMR and other systems.
• Complete monthly reporting to ensure program compliance.
• Attend training as assigned.
