Vynca Inc. is seeking a Lead Care Manager to coordinate client care across health and social services. The role partners with providers, families, and care teams to ensure timely, cost-effective access to services and supports.

Qualifications

• 2+ years experience in care management, care navigation, or community health work serving vulnerable populations.
• Willing to work Monday–Friday 8:30am–5:00pm PT with some evenings/weekends possible.
• Knowledge of government and community resources related to social determinants of health.
• Clean driving record and reliable transportation.
• Excellent oral and written communication skills.
• Strong interpersonal skills and teamwork.
• General computer skills including Google Workspace, MS Office and internet usage.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative, memory and trauma-informed care.
• Develop client care plans and set SMART goals.
• Provide services where the member resides, including telehealth or field-based care.
• Connect clients to social services and supports as needed.
• Advocate with healthcare professionals on behalf of the client.
• Utilize evidence-based practices such as Motivational Interviewing and Trauma-Informed Care.
• Complete documentation and monthly program reporting.
• Maintain up-to-date health records in EMR and other systems.
