Job summary

Vynca Inc. is seeking a Lead Care Manager (LCM) to coordinate complex care for clients across health systems and community partners.

Based in a remote capacity, you will partner with physicians, social services, and families to build person-centered care plans and ensure timely access to benefits and services. You’ll guide an ECM team, align with the Director and program managers, and drive outcomes through evidence-informed practices while maintaining comprehensive documentation and program

Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
• Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for potential evenings and weekends.
• Working knowledge of government and community resources related to social determinants of health.
• Clean driving record, valid driver's license, and reliable transportation.
• Excellent oral and written communication skills.
• Positive interpersonal skills required.
• Must have general computer skills and a working knowledge of Google Workspace, MS Office and the internet.
• Bilingual (English/Spanish)

Responsibilities

• Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral to community-based services.
• Oversees the development of the client care plans and goal setting.
• Offer services where the member resides, seeks care, or finds most accessible (office, telehealth, or field).
• Connect clients to other social services and supports as needed.
• Advocate on behalf of the client with health care professionals (e.g. PCP).
• Utilize evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Conduct outreach and engagement to facilitate linkage to the ECM program and log activity in CRM.
• Evaluate client progress and update SMART goals.
• Provide mental health promotion.
• Arrange transportation (e.g., ACCESS).
• Complete all documentation, including outcome measures within care plans timelines.
• Maintain up-to-date patient health records in EMR and other systems.
• Complete monthly reporting to ensure program compliance.
• Attend training as assigned.
