Job summary

Vynca is seeking a Lead Care Manager (LCM) to coordinate comprehensive care for clients with complex needs. The role reports to ECM leadership and partners with healthcare providers, social services, and families to ensure timely, person-centered care across settings.

The LCM will oversee care plans, coordinate benefits, and advocate for clients while documenting activities and maintaining EMR records. Remote/field-based care may be involved.

Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
• Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for potential evenings and weekends.
• Working knowledge of government and community resources related to social determinants of health.
• General computer skills and working knowledge of Google Workspace, MS Office and the internet.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referrals to community-based services.
• Oversee development of client care plans and goal setting.
• Offer services where the member resides, including office-based, telehealth, or field-based.
• Connect clients to social services and supports as needed.
• Advocate for clients with healthcare professionals using evidence-based practices.
• Conduct outreach and engagement activities and log activity in CRM.
• Complete documentation and outcome measures within care plans timelines.
• Maintain up-to-date patient health records in EMR and other systems.
• Attend training as assigned.
