Job summary

Vynca Inc. is seeking a Lead Care Manager (LCM) to coordinate care for clients with complex needs. You will partner with healthcare providers, social services, and family caregivers to ensure timely, cost-effective, and person-centered care.

The LCM reports to ECM leadership and drives care planning, engagement, and documentation in a dynamic home-based care program. The role focuses on assessing needs across health domains, developing care plans, and enabling access to services through office,

Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
• Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for potential evenings and weekends.
• Working knowledge of government and community resources related to social determinants of health.
• General computer skills and working knowledge of Google Workspace, MS Office and the internet.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and linkage to services.
• Oversee development of client care plans and goal settings.
• Provide services where the member resides, including office-based, telehealth, or field-based care.
• Connect clients to social services and supports as needed.
• Advocate for clients with health care professionals using evidence-based practices.
• Conduct outreach and engagement to facilitate linkage to the ECM program and log activity in the CRM.
• Complete all documentation and outcome measures within established care plan timeframes.
• Maintain up-to-date patient health records in the EMR and other systems.
• Attend training as assigned.
