Job summary

Vynca Inc. is seeking a Lead Care Manager (LCM) to coordinate complex care for individuals, collaborating with healthcare providers, social services, and family caregivers to ensure timely and cost-effective access to services.

The LCM will manage cases, develop care plans, and advocate for patients across office-based, telehealth, and field-based settings, while documenting outcomes and maintaining EMR records.

Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
• Willingness to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for potential evenings and weekends.
• Working knowledge of government and community resources related to social determinants of health.
• General computer skills and working knowledge of Google Workspace, MS Office and the internet.
• Bilingual English/Spanish preferred

Responsibilities

• Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referrals to community services.
• Oversee development of client care plans and goal settings.
• Provide services where the member resides, seeks care, or is most accessible (office, telehealth, field).
• Connect clients to social services and supports as needed.
• Advocate with health care professionals using evidence-based practices (Motivational Interviewing, Harm Reduction, Trauma-Informed Care).
• Conduct outreach to facilitate linkage to ECM program and log activity in CRM.
• Complete all documentation including outcome measures within care plans timeframes.
• Maintain up-to-date patient health records in EMR and other systems.
• Attend training as assigned.
