Qualifications

• 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
• Proficient with Google Workspace, MS Office, and common CRM/EMR systems.
• Ability to work from home or field settings; patient-centered and compassionate.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative and memory care.
• Oversee development of client care plans and goal setting.
• Offer services where the member resides, including telehealth, field-based care, or office visits.
• Connect clients to social services and supports as needed.
• Advocate for clients with health professionals using evidence-based practices.
• Complete documentation and log activity in the CRM.
