Job summary

Vynca Inc. is seeking a Lead Care Manager to coordinate care for clients, partnering with providers, social services, and families to ensure seamless, person-centered care.

You will develop and monitor care plans, coordinate benefits, and promote wellness while documenting progress in EMR/CRM systems. This remote role requires 2+ years in care management and strong communication skills, with available hours Monday–Friday PT.

Qualifications

• 2+ years of experience in care management or equivalent role.
• Willing to work Monday–Friday 8:30am–5:00pm Pacific Time with potential evenings/weekends.
• Knowledge of government and community resources related to social determinants of health.
• Clean driving record and reliable transportation.
• Excellent oral and written communication skills; proficient with Google Workspace and MS Office.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, and social supports.
• Develop and monitor client care plans and SMART goals.
• Provide services where the member resides, including telehealth or in-person visits.
• Coordinate with social services and healthcare providers to link to needed resources.
• Advocate for clients with health professionals and care teams.
• Document outcomes and progress in EMR/CRM systems; complete monthly reporting.
