Job summary

Molina Healthcare, Inc. is seeking an IRIS Consultant (IC) to build relationships with participants in the Wisconsin IRIS program.

This remote role includes regular home visits, helping participants achieve goals in employment, housing, health, and community involvement. Successful candidates will be compassionate, resourceful partners who can navigate IRIS budgets, coordinate with providers, and maintain strict confidentiality.

Qualifications

• At least 2 years experience in health care, preferably in care coordination.
• Bachelor’s degree as specified or four years commensurate experience.
• Valid and unrestricted driver’s license and reliable transportation for job-related travel.
• Ability to develop positive and effective work relationships with coworkers, clients, participants, providers, regulatory agencies and vendors.
• Ability to work independently with minimal supervision and demonstrate self-motivation.
• Demonstrated knowledge of long-term care programs.
• Familiarity with self-determination principles.
• Excellent time-management and prioritization skills.

Responsibilities

• Meet IRIS participants in person at least four times per year, including one annual visit in the home.
• Provide program orientation to new participants and explain rights, responsibilities, and paperwork.
• Develop customized IRIS plans addressing employment, housing, health, safety, and community goals.
• Document orientation and planning activities in the IRIS data system (WISITs) within 48 business hours.
• Educate participants on budget reports and coordinate with the Fiscal Employer Agency as needed.
• Maintain confidentiality and HIPAA compliance; work with IRIS Consultant Agency staff for successful implementation.
