Job summary

Molina Healthcare is seeking an IRIS Consultant (IC) to build relationships with program participants and help them navigate the Wisconsin IRIS program. This remote role requires occasional home visits to participants and regular coordination with the IRIS Consultant Agency to develop customized plans addressing employment, housing, health, and safety goals.

Qualifications

• 2+ years experience in health care, care coordination, or IRIS-related work; 1+ year serving target groups.
• Bachelor's degree in social work, psychology, human services, counseling, nursing, education, or related field (or 4 years commensurate experience).
• Valid and unrestricted driver's license and reliable transportation for travel requirements.

Responsibilities

• Meet in person with IRIS participants at least four times per year, including one annual home visit.
• Provide program orientation to new participants, outlining rights, responsibilities, and budget use.
• Identify participant goals and develop IRIS plans to achieve outcomes within the IRIS budget.
