Job summary

Molina Healthcare is seeking an IRIS Consultant to remotely support Wisconsin participants in the IRIS long-term care program. The IC will visit participants in their homes regularly and work with them to maximize benefits, form budgets, and connect to community resources.

The role emphasizes relationship-building, compassionate support, and collaboration with IRIS staff to implement individualized care plans and ensure participant goals are met within program rules.

Qualifications

• Minimum 2 years in health care, preferably care coordination
• 1 year experience with IRIS program target groups or equivalent
• Valid driver’s license and reliable transportation

Responsibilities

• Build relationships with IRIS participants and partners
• Help navigate Wisconsin IRIS program to meet participant goals
• Develop personalized IRIS plans for employment, housing, health, safety, and community involvement
• Document orientation and planning activities in the IRIS data system (WISITs) within 48 business hours
• Research community resources and supports aligned to participant outcomes
• Coordinate with Fiscal Employer Agency and IRIS Consultant Agency on provider billing and related tasks
• Attend monthly in-person team meetings and participate in regular check-ins
