Job summary

Molina Healthcare is hiring an IRIS Consultant (IC) for a remote role focused on Wisconsin’s IRIS program. The IC will partner with participants in their communities, meeting in person up to four times per year and supporting residents in navigating long‑term care options.

The role emphasizes compassionate relationship building, plan development for goals across employment, housing, health, and safety, and collaboration with IRIS staff to ensure successful program implementation.

Qualifications

• At least 2 years experience in health care, preferably in care coordination.
• Bachelor’s degree in a social work, psychology, human services, counseling, nursing, special education, or a closely related field.
• Valid and unrestricted driver’s license and reliable transportation for travel requirements.

Responsibilities

• Build relationships with participants and help them navigate the Wisconsin IRIS program.
• Develop customized IRIS plans for achieving participants' goals related to employment, housing, health and safety.
• Document orientation and planning activities in the IRIS data system within 48 business hours.
• Educate participants on budget reports and act as liaison between the Fiscal Employer Agency and the IRIS Consultant Agency.
