Job summary

Molina Healthcare is seeking an IRIS Consultant (IC) to support participants enrolled in the Wisconsin IRIS program. This fully remote role involves home-visit coordination, budget guidance, and collaboration with community resources to meet participants’ long-term care goals.

You will build trusted relationships, conduct in-home visits four times a year, and help navigate the self-directed IRIS program while maintaining HIPAA confidentiality and strong documentation practices.

Qualifications

• Minimum 2 years in health care, care coordination preferred.
• Bachelor’s degree in social work, psychology, human services, nursing or related field (or four years’ equivalent).
• Valid driver’s license and reliable transportation for job-related travel.

Responsibilities

• Meet in person with IRIS participant at least four times per year, including one annual home visit.
• Provide program orientation to new participants and explain rights/responsibilities of IRIS participants.
• Develop individualized IRIS plans to achieve participant goals (employment, housing, health, etc.).
• Document all activities in the IRIS data system (WISITs) within 48 business hours of visits.
• Educate participants on budget reports and coordinate with Fiscal Employer Agency and IRIS Consultant Agency.
