Job summary

Molina Healthcare is seeking an IRIS Consultant to support Wisconsin residents enrolled in the IRIS program. This remote role requires regular visits with participants in their homes and coordinating care in the community.

As an IC, you will help participants set personal outcomes, navigate long-term care options, document progress in the IRIS data system, and connect them to resources. Strong communication, organization, and knowledge of self-determination are essential.

Qualifications

• At least 2 years in health care, preferably care coordination, with IRIS-type experience.
• Bachelor’s degree in a social work, psychology, human services, counseling, nursing, or closely related field.
• Valid driver’s license and reliable transportation for job-related travel.
• Ability to work independently with minimal supervision.
• Knowledge of long-term care programs.
• Familiarity with self-determination principles.
• Excellent time-management and communication skills.
• Proficient with Microsoft Office.

Responsibilities

• Meet IRIS participant in person at least four times per year, with one annual home visit.
• Provide program orientation to new participants and explain rights and responsibilities.
• Identify personal outcomes and ensure progress within IRIS budget and DHS requirements.
• Document all orientation and planning activities in the IRIS data system within 48 business hours.
• Research community resources and supports for participants and share information.
• Document progress and changes to IRIS plans as needed.
• Assist with budget-related paperwork and liaise with Fiscal Employer Agency.
• Develop and maintain engaged relationships with participants and communicate program changes.
• Maintain confidentiality and HIPAA compliance.
• Coordinate with IRIS Consultant Agency staff for successful plan implementation.
• Attend monthly in-person team meetings and participate in weekly check-ins.
