Job summary

Molina Healthcare is seeking an IRIS Consultant to support participants in the Wisconsin IRIS Program. This remote role involves home-based outreach with regular in-person visits and community engagement to help participants achieve personal goals within IRIS budgets.

You will build trusted relationships to navigate long-term care options and access community resources. Ideal candidates have health care/care coordination experience, strong communication, and the ability to work independently

Qualifications

• At least 2 years experience in health care, preferably in care coordination.
• At least 1 year of experience serving IRIS program target groups or equivalent experience.
• Bachelor’s degree in a social/health field or four years of commensurate experience if no degree.
• Valid and unrestricted driver’s license and reliable transportation.
• Excellent verbal and written communication skills.

Responsibilities

• Meet IRIS participants in person a minimum of four times per year, with one annual home visit.
• Provide program orientation and explain rights and responsibilities in IRIS.
• Identify personal outcomes and ensure progress within IRIS budget and DHS rules.
• Document orientation and planning activities in WISITs within 48 business hours.
• Research community resources and share information with participants.
• Document progress/changes in plans as requested by participants.
• Educate participants on reading monthly budget reports and assist with billing/payments.
• Maintain confidentiality and HIPPA compliance.
• Collaborate with IRIS Consultant Agency staff and attend monthly meetings.
