Job summary

Molina Healthcare is seeking an IRIS Consultant (IC) to work remotely in Wisconsin, partnering with IRIS participants in their home and community. You will build relationships, help navigate the Wisconsin IRIS program, and develop customized plans to achieve goals in employment, housing, health, and safety.

The role emphasizes compassionate, reliable support, in-person participant meetings, budgeting guidance, and collaboration with the IRIS team.

Qualifications

• 2+ years health care experience, preferably in care coordination.
• Bachelor’s degree in a related field or four years of commensurate experience.
• Valid driver's license and reliable transportation for job travel.
• Ability to work independently and build relationships with diverse participants.
• Knowledge of long-term care programs and self-determination principles.
• Strong time-management and prioritization skills.

Responsibilities

• Meet IRIS participants in person at least four times per year, including one annual home visit.
• Provide program orientation; discuss rights, responsibilities, and budgeting.
• Identify participant goals and develop customized IRIS plans.
• Document orientation and planning activities in the IRIS data system within 48 business hours.
• Research community resources and supports to fit participant outcomes.
• Educate participants on monthly budget reports and assist with provider billing and budgeting.
• Collaborate with IRIS Consultant Agency staff and attend monthly team meetings.
