Job summary

Molina Healthcare is seeking an IRIS Consultant to partner with participants enrolled in the Wisconsin IRIS program. This remote role requires regular in-person visits in participants' homes and communities, building trusted relationships to help achieve long-term care goals.

You will educate participants on budget management, coordinate resources, and document planning activities within the DHS IRIS framework, ensuring confidentiality and HIPAA compliance.

Qualifications

• 2+ years in health care, care coordination preferred.
• Bachelor’s degree in social work, psychology, human services, nursing or related field (or 4 years relevant experience).
• Valid driver’s license and reliable transportation for travel.
• Ability to work independently and manage multiple projects.

Responsibilities

• Build relationships with IRIS participants and navigate the Wisconsin IRIS program.
• Develop customized IRIS plans for goals related to employment, housing, health, safety, and community involvement.
• Educate participants on budget reports and plan management within IRIS budget constraints.
• Document orientation and planning activities in the IRIS data system (WISITs) within 48 hours.
• Collaborate with IRIS Consultant Agency staff and attend monthly in-person team meetings.
