Job summary

Molina Healthcare is seeking an IRIS Consultant (IC) to support Wisconsin IRIS program participants. This remote role requires regular in-person visits to participants' homes and communities to navigate and optimize long-term care plans.

You will build trusting relationships, coordinate resources, and document progress in the IRIS data system while ensuring HIPAA compliance. Travel and collaboration with agency staff are part of the role.

Qualifications

• 2+ years experience in health care, preferably care coordination.
• Bachelor’s degree in social work, psychology, human services, counseling, nursing, education, or related field.
• Valid and unrestricted driver’s license and reliable transportation for travel.
• Ability to work independently with minimal supervision and manage multiple projects.

Responsibilities

• Meet IRIS participants in person at least four times per year, including one annual home visit.
• Educate participants on rights, responsibilities, and budget use within the IRIS program.
• Develop individualized IRIS plans to achieve goals related to employment, housing, health, safety, and community involvement.
• Document all activities in the IRIS data system within 48 business hours of visits.
