Sentara Health is seeking an Integrated Case Manager, RN. This role is remote but requires in-person assessments; travel to Roanoke and northern areas is needed up to 60-90 minutes to see members about 50-75% of the week.

You will develop, monitor, and revise patient care plans across the care continuum, performing telephonic clinical assessments and coordinating with the health care team to optimize outcomes.
