Job summary

Vynca Inc. is seeking an Intake Coordinator to support ECM Sales/Marketing and Clinical Outreach Specialists in organizing and enrolling patients in our programs.

You will build positive relationships with teams and families, assist in outreach to educate clients about offerings, and facilitate enrollment with a patient-centric approach. The role emphasizes organization, effective communication, and ability to review medical charts; prior healthcare experience is preferred.

Qualifications

• Minimum of 2 years of intake/admission experience in a high call volume environment.
• CMA/CCMA certificate issued by a recognized group.
• Prior experience in a remote role with a quiet, secure workspace.
• 2+ years in palliative care, home health, hospice, or discharge planning.
• 2+ years working with large safety net hospitals regionally (preferred).
• Willing to work Mon-Fri 8:30am-5:00pm PT; potential evenings/weekends.

Responsibilities

• Facilitates intake, admission, and outreach to enroll new patients.
• Assesses patient status during admission and maintains documentation.
• Keeps databases, logs, census sheets, and contact lists up to date.
• Completes insurance eligibility checks and assigns clients to the Clinical Outreach Specialist.
• Coordinates enrollment workflow with sales, outreach, care delivery, and RCM.
• Maintains all enrollment documentation and records interactions in the system.
• Attends staff and census meetings; participates in outreach team meetings.
• Meets or exceeds departmental KPIs and supports organizational needs.
• Answers incoming calls and emails.
