Vynca Inc. is seeking an Intake Coordinator to support ECM Sales/Marketing and Clinical Outreach teams in enrolling patients into our programs.

The role focuses on coordinating intake, reviewing medical charts, and building strong relationships with patients and internal teams. The ideal candidate will have at least 2 years in intake/admission in a high‑volume setting, CMA/CCMA certification, and experience with Google Suite and Salesforce.

Qualifications

• Minimum 2 years of intake/admission experience in a high‑volume environment.
• CMA/CCMA certificate issued by a recognized group.
• Remote work requires a quiet, secure workspace to protect privacy.
• 2+ years in palliative/home health/hospice or discharge planning or case management, highly preferred.
• Experience with safety net hospitals in the region is preferred.
• Willing to work Monday–Friday 8:30am‑5:00pm PT, with evenings/weekends as needed.
• Strong cultural competency with the target population.
• Fluent in English; bilingual Spanish preferred.
• Training alongside social workers/nurses/MAs or related clinical exposure.

Responsabilités

• Facilitates intake, admission, and outreach to enroll incoming patients.
• Provides ongoing assessment of patient status during admission.
• Maintains databases, logs, census sheets, and resource lists; updates sheets.
• Performs insurance eligibility checks and assigns clients to Outreach Specialist.
• Collaborates with sales, outreach, care delivery, and RCM to support enrollment workflow.
• Documents all enrollment interactions in the operating system.
• Attends staff, census, and outreach meetings; participates in weekly team meetings.
• Meets or exceeds departmental KPIs; performs additional duties as needed.
• Answers incoming calls and emails.
