Qualifications

• 2+ years of experience in Behavioral Health, Social Services or related field.
• Willing to travel 50–75% in Cook County, Illinois (North/Northwest Chicago).
• 2+ years of electronic documentation experience and MS Office proficiency.

Responsibilities

• Evaluate members’ needs and eligibility and recommend case resolution pathways.
• Identify high-risk factors and coordinate with clinical teams.
• Coordinate and monitor care plan activities and progress.
• Consult with Medical Directors and other programs to overcome barriers to care.
• Provide coaching and support to empower members in healthcare decisions.
