Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willing to travel 50-75% in Cook County, Illinois (North/Northwest Chicago).
• 2+ years of electronic documentation experience and proficiency with Microsoft Office applications.

Responsibilities

• Evaluate members' needs and determine care plan based on benefits and available programs.
• Identify high risk factors and coordinate appropriate referrals to clinical teams.
• Coordinate and monitor progress of assigned care plans.
• Collaborate with Medical Directors and other programs to improve care quality and outcomes.
• Escalate quality of care issues through established channels.
• Assist members to access benefits and necessary services.
