Job summary

CVS Health Corporation is seeking a Case Management Coordinator for a full-time telework position serving members in Cook County, Illinois, with 50-75% travel around the South Side of Chicago and Southern Cook County.

You will evaluate member needs, coordinate and monitor care plans, educate members about benefits, and collaborate with clinical teams to promote optimal health outcomes while maintaining regulatory compliance.

Qualifications

• 2+ years in Behavioral Health or Social Services.
• Experience with electronic documentation.
• Proficiency with Microsoft Office applications.

Responsibilities

• Evaluate members’ needs and eligibility; recommend care resolution.
• Identify high risk factors and coordinate referrals to case management.
• Coordinate and monitor care plan activities and progress.
• Collaborate with Medical Directors and case managers to improve quality of care.
• Escalate quality issues through established channels.
• Negotiate options to meet members’ benefits and healthcare needs.
• Coach members to engage in healthy lifestyle choices.
• Participate in team meetings and mentor peers.
• Assist members in healthcare decision-making with providers.
• Document care in compliance with regulatory guidelines.
