Responsibilities

Evaluate members’ needs/eligibility and recommend care resolution paths.
Identify high-risk factors and coordinate with clinical teams.
Coordinate and monitor assigned care plan activities and progress.
Collaborate with Medical Directors and health programs to address barriers to goals.
Escalate quality of care issues through established channels.
Engage members to promote lifestyle changes and participation in decisions.
