Job summary

CVS Health is seeking a Case Management Coordinator to support members in Cook and surrounding counties with scheduling, benefits access and education using care management tools. You will collaborate with case managers, providers and community organizations to achieve optimal outcomes, address social determinants, and help members navigate care decisions.

This full‑time role requires a bachelor’s or non‑licensed master in behavioral health or human services, strong analytical and communication

Qualifications

• Bachelor's degree or non-licensed master's in behavioral health or human services.
• 2+ years in behavioral health, social services or related field.
• Case management and discharge planning experience preferred.
• Managed Care experience preferred.

Responsibilities

• Evaluate members' needs and eligibility using care management tools and data.
• Identify high risk factors and refer to clinical case management as needed.
• Coordinate and monitor assigned care plan activities.
• Engage with colleagues in team meetings and mentoring.
• Assist members with scheduling and understanding benefits and education.
