Qualifications

• Bachelor's degree in behavioral health or related field.
• 2+ years experience in case management, discharge planning or related field.
• Experience with managed care preferred.
• Ability to travel within a geographic area for in‑person activities.
• Excellent analytical and problem‑solving skills.
• Strong communication, organizational and interpersonal skills.
• Proficiency with standard corporate software (MS Word/Excel/Outlook/PowerPoint).
• Independent, self-motivated and comfortable collaborating virtually.

Responsibilities

• Evaluate member needs and eligibility to develop care plans.
• Coordinate and monitor care plans and outcomes.
• Identify high-risk factors and coordinate with clinical teams.
• Assist with scheduling, benefits access and member education.
• Engage with members to support informed healthcare decisions.
• Document care activities in compliance with guidelines.
