Qualifications

• Be an RN or LPN or
• 5+ years independent Support Coordinator or case manager for 1915(c) HCBS Waiver
• 4-year or higher degree in human services or health care
• Meet federal requirements for a QDDP or QIDP

Responsibilities

• Engage members to assess medical, behavioral, functional, cultural and socioeconomic needs
• Develop and implement person-centered care plans
• Coordinate with internal care teams and community resources
• Provide referrals and linkage to providers and housing etc.
