Job summary

UnitedHealthcare is seeking a Field Care Coordinator in Tennessee to manage a panel of members in the EC F CHOICES program. The role blends home-based work with field responsibilities and requires close collaboration with care teams to assess medical, behavioral, and social needs.

Responsibilities include creating and implementing person-centered care plans, coordinating with providers, and making referrals for housing, transportation, and supportive services.

Qualifications

• Degree or RN/LPN or QDDP/QIDP or 5+ years HCBS coordination experience.
• Valid driver’s license and ability to travel up to 75% within counties.
• Reside within 50 miles of Shelby and surrounding counties, TN.

Responsibilities

• Engage members face-to-face or telephonically to assess needs.
• Develop and implement person-centered care plans addressing medical, behavioral, and social needs.
• Coordinate with internal team and community resources to implement plans.
• Provide referrals to housing, pharmacy, and other supports as appropriate.
• Support appropriate utilization of services and improve care quality.
