Job summary

UnitedHealth Group is seeking a Field Care Coordinator to manage a panel of members with medical and behavioral needs from a home-based office in Roswell, NM. The role requires comprehensive needs assessments, care plan development, and collaboration with care teams to promote appropriate service utilization and quality of care.

The position offers full-time hours, with flexibility to work remote when not in the field and potential travel within the territory.

Qualifications

• Bachelor's degree OR 2+ years of relevant health care experience.
• 2-year degree or higher with 3+ years of clinical experience.
• Current, unrestricted licensure as RN or a licensed social/mental health professional with 3 years clinical experience.
• 5+ years of relevant experience, including 3+ years of clinical experience.
• 4+ years in a clinical environment (1+ year as Care Coordinator).
• LPN with 3+ years of clinical experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Driver's license and ability to travel up to 50% within territory.
• Designated home workspace with high-speed internet.
• Reside in or near Roswell, NM.

Responsibilities

• Engage members face-to-face and/or telephonically for CNA and needs assessment.
• Develop and implement person-centered care plans for medical/behavioral needs.
• Collaborate with care team, providers, and community resources to execute plans.
• Provide education and coaching to support self-management of care needs.
• Support discharge planning and care transition after ER/inpatient/SNF.
• Advocate for members and ensure needs and choices are represented.
