Job summary

UnitedHealth Group's UnitedHealthcare division seeks an ECF CHOICES Support Coordinator to manage a panel of members with intellectual or developmental disabilities. You will perform needs assessments, create care plans, and coordinate with the care team to improve health outcomes.

The role is home-based with 75% field responsibilities, requiring travel within Tennessee. All telecommuting is possible within a 50-mile radius of Rutherford County, TN, enabling flexible remote work as needed.

Qualifications

• Meet one of the following: RN or LPN or 4-year degree in human services or related field or QDDP/QIDP.
• Reside in or within 50-mile radius of Williamson County, TN.
• Valid US driver's license and ability to travel up to 75% within counties to meet with members.

Responsibilities

• Engage members to complete comprehensive needs assessments including medical, behavioral, functional, cultural, and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, and social determinants of health.
• Collaborate with care team, providers, and community resources to implement care plans.
• Provide referrals to housing, pharmacy, peer support and community providers as appropriate.
