Job summary

UnitedHealth Group seeks an ECF CHOICES Support Coordinator to manage a panel of members with developmental disabilities, focusing on medical, behavioral and social needs. The role blends in-home field work (75%) with remote coordination and requires community engagement and cross-team collaboration.

You will develop person-centered care plans, coordinate with internal and external providers, and link members to services to promote quality of care and improved outcomes.

Qualifications

• 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or other human service or health care profession
• RN or LPN
• QDDP or QIDP certification
• 5+ years as independent Support Coordinator or case manager for 1915(c) HCBS Waiver with POM/Assessment trainings

Responsibilities

• Conduct comprehensive needs assessments covering medical, behavioral, functional, cultural and socioeconomic needs
• Develop and implement person-centered care plans for chronic conditions, health promotion and social determinants of health
• Coordinate with care team, providers and community resources to implement care plans
• Provide referrals and linkages to Housing Navigator, Pharmacy Team, Peer Specialist and other resources
