Job summary

UnitedHealth Group is seeking a Field Care Coordinator to manage a panel of members with low-to-medium complexity medical and behavioral needs. You will assess medical, behavioral, and social factors, develop care plans, and collaborate with providers to improve outcomes.

The role is full-time, 40 hours weekly, with a home-based office and field travel. Remote work eligibility applies to NM residents within commuting distance of Roswell.

Qualifications

• Bachelor's degree OR 2+ years of relevant health care experience.
• Two-year degree or higher with 3+ years of clinical experience.
• Current, unrestricted independent licensure as RN or Social Work/Mental Health licensure with 3 years of clinical experience.
• 5+ years of relevant experience including 3+ years in clinical setting.
• 1+ years of experience with MS Office, including Word, Excel, and Outlook.
• Must have a driver's license and be able to travel up to 50% within territory.
• Reside in or near Roswell, NM.

Responsibilities

• Engage members face-to-face and/or telephonically for comprehensive needs assessments.
• Develop and implement person-centered care plans for chronic conditions and social determinants of health.
• Coordinate with care teams, providers, and community resources to execute care plans.
• Provide education and coaching for member self-management of care needs and lifestyle changes.
• Support discharge planning and care transition following ER visits or SNF admission.
• Advocate for members to ensure needs and choices are represented.
